To apply for a loan:
  1. Complete and print the loan application.
  2. Print and sign the credit authorization.
  3. If you are self-employed, print and complete the Request for Transcript of Tax Returns.
  4. Fax or mail all documents to your mortgage consultant.

Thank you for your interest in being prequalified for a mortgage loan through First Bank’s prequalification site. We are committed to responding to all our customers quickly. Upon submission of your request, a Loan Consultant will be in touch with you as soon as possible to discuss your mortgage options and answer your questions.

Helpful Hints for using this site:
  • To protect your personal information, after 90 minutes of inactivity the site will close without saving your information. You will be required to begin a new session.
  • First Bank is committed to the responsible use of and protection of your customer data. The site you are about to enter is a secure location to safeguard the privacy of your nonpublic information.
  • Be careful when using public computers to enter your personal information and always be sure to log off as instructed. When in doubt, contact your Loan Consultant directly. We cannot guarantee your privacy on a public computer.
  • If you don’t already have a contract to purchase a property, just use the term “TBD” in the address line for the street address. We do need to have a city, state and zip code for the area in which you are interested.
  • Upon completion and submission, we will send you a confirming email.
  • Please complete all data fields. You may either go directly to the prequalification page or first review a checklist of income and asset documents that will assist in completing your prequalification request. Please choose the appropriate option below.

Document Checklist

Employment Income Documents

If you're salaried:
  • W-2 forms for the most recent 2 years
  • Payroll stubs for the past 30 days (showing year-to-date earnings)
  • Federal Tax returns for the past 2 years with all schedules
If you’re self-employed:
  • Year-to-date income and expense documentation
  • Federal Tax returns for the past 2 years

Non-Employment Related Income Documents

Rental Income:
  • Rental agreements
Retirement/Social Security:
  • Award letter, tax returns, or bank statements if direct-deposit
Alimony/Child Support:
  • Divorce decree/separation agreement (cancelled checks may be necessary)
Other Income:
  • Provide appropriate documentation as discussed with your Loan Consultant
Assets:
  • Most recent two months bank statements including checking, savings, and money markets. The source of any non-payroll deposits must be provided.
  • Any mutual funds, stocks, and/or any retirement accounts, both personal and business
Explanation Letters:
  • Explain any “non-payroll” deposits on asset statements
  • Explain any past derogatory credit
General Documentation:
  • Authorization and IRS 4506T to be signed
  • Copy of current Photo Identification
Already Under Contract:
  • Purchase/deposit agreement (signed by buyer and seller)
  • Contact information for your homeowner’s hazard insurance policy
Once Your File Is In Process:
  • You agree to provide any additional required documentation within 24 hours from time requested.
